My boots fit as well or better than any other boots I have owned, they are Vasque Bitteroots. but I live 30 k (20 miles) from the nearest hill. So when I first travelled to Mount Walsh on the descent my toes hit the end of my boot due to the extreme grade. If it had been a larger descent I probably would have ended up with black toes.
Looking at the Gear Institutes assessment of the boot Marcus Woolf mentions the third eyelet as difficult to use. After a quick search on the net I found a post by Philip Werner whom I follow on twitter and on Google +, His post was on boot lacing and provided a wealth of information.
